There will be a meeting for all laid off members on Monday, 8/25/08 at
9:30am at the Local 402 Union Hall. At this meeting there will be
representatives from the following:
Work Plus One-Stop (ODJFS)
AFL-CIO Rapid Response
United Way
Benefits Plus
Neighborhood Housing Partnership
Clark State
Members are strongly encouraged to be present at this meeting, so that they
can receive information and assistance pertaining to financial and
educational funding that is available to our membership.

All members laid off effective 8/22/08 will be able to pick up their checks on Thursday 8/28/08 from 1:00pm — 3:00pm at the employment office.

We would also like to say "Congratulations and Thank You" to the active and retired membership and others on the successful food drive for the Second Harvest Food Bank. Over 775 lbs. of food and $1,101.46 was donated on Wednesday. Many of our laid off members and retirees as well as other in need within our community have and are using this program. The administrators of Second Harvest were very appreciative of our generosity
and Thank All of You.

IMPORTANT INSTRUCTIONS FOR FILING YOUR CLAIM
For Employees of: International Truck and Engine

Last Day Worked: August 22, 2008
The following mass layoff number is being provided to you for use when filing your Application for Determination for Unemployment Benefit Rights. This number may be used only for the application filed because of your layoff on the last day of work shown above.
MASS LAYOFF NUMBER: 800571 (This number may be used only for this layoff.)
To file your application for Determination of Unemployment Benefits, take the following action starting August 24 . 2008 but no later than August 30, 2008:
File online at: http://unemployment.ohio.gov (accessible 24 hours a day, 7 days a week). Limited service may be available while our system is being updated nightly. Check the Website for available services. Be sure to follow the prompts given.
If you do not have internet access: Call 1-877-644-6562 (business hours are 7:30 AM - 4:30PM, Monday - Friday, except holidays). Peak call times are Monday and Friday. For faster service call Tuesday, Wednesday, Thursday or file online. Limited service is available Sunday evenings.
You should be prepared to provide the following information:
> Your social security number and either your driver license or state ID number
> Your name, address, telephone number, and e-mail address (if applicable)
> The name, address, telephone number, and dates of your employment with each employer for whom you worked during the past 6 weeks of employment
> If you had out-of-state employment within the past 18 months; the name, address, telephone number and dates of your employment for each employer with whom you worked out of state during the past 18 months
> If you were separated from military service with any branch of the U.S. armed forces within the past 18 months , your discharge papers (form DD-214, member 4)
> If you were employed by the federal government within the past 18 months, the SF-8 and/or SF-50 form given to you by your government employer at the time of your separation
> Dependents' names, social security numbers and dates of birth
> If claiming dependents, your spouse's name, social security number, and birth date
> Alien Registration Number and the expiration date of your work authorization (if you are not a US citizen or National)
> Direct Deposit Information (if preferred, you will need your bank routing number and account number).
Once your application has been filed, you will receive further information in the mail.
You must access the ODJFS website or call ODJFS twice to receive benefits. First, you must make application for benefits as shown above. Next, you will need to access your claim again, either by the web at http://unemployment.ohio.gov or by the interactive voice response (IVR) system 1-877-6446562, to file a claim for benefits for week(s) of unemployment. If you do not return to work, you will need to continue to claim weeks of unemployment this way.

Complete the entire top portion of the SUB form that is outlined in the dark bold square only

Keep the back pink copy for your records

Do NOT remove the carbon paper

Do not staple, tape or paperclip your packet. Lift the top page and slip your paperwork inside. This saves us valuable time.

Waiting Week: Must have determination and proof of waiting week filing and 0.00 benefits paid.

You may file your SUB application immediately after the week ending date you are claiming.

If you are working while on layoff, you must provide a copy of your check stub relative to the period being claimed.

If you are denied unemployment benefits, you must provide a copy of your denial and a copy of your SCOTI form with each application submitted.

You can drop off your application in the Employment Lobby with outside access from 7:00 to 3:15 weekdays or you can mail it to; 6125 Urbana Road, Springfield, Ohio 45502 Attention: SUB.

Applications must be received by the end of the day on Tuesdays. (We will try to do any we receive on Wednesdays as time permits.)

SUB checks are issued on Fridays. Please allow time for your check to be deilvered.
TO ALL 402 MEMBERS INVOLVED IN THE AUGUST 22, 2008 LAY-OFF
MAKE SURE YOU READ THE INSTRUCTION SHEET!!!!
1. To be eligible for State Benefits and SUB you must file timely and within time limits!!!
2. To be eligible for SUB you must file for State Unemployment or have other qualifying compensation (Outside earnings).
3. You may qualify for SUB benefits even if you do not qualify for State benefits.
4.. The State Bureau mandates that you file by phone at 1-577-644-6562 or online at http://unemployment.ohio.gov. Not before August 10, 2008 and no later than August 16, 2008 or you could be denied Benefits!
5. YOU MUST CALL ODJFS OR ACCESS THE WEBS1TE ONCE DURING THE FIRST WEEK OF LAY-OFF TO OPEN A CLAIM AND YOU MUST CONTACT THE ODJFS WITHIN 21 DAYS OF THE END OF THAT WEEK IN ORDER TO RECEIVE BENEFITS/ WAITING WEEK FOR THAT WEEK!!
6. If you establish a new claim, you will have to serve a waiting week. To file for SUB you need a copy of the Determination from the State of Ohio. Also for the waiting week you will need a copy of your check stub that shows the amount of $0.00 for the waiting week. if this is not a waiting week, you must submit a copy of your Benefit amount deposited to your account or to your US BANK Card.
7. If you qualify for state benefits, the maximum benefits are as follows:
New Claim after 01/01/07
$355- single applicant
$431- applicant with 1-2 dep.
$479- applicant with 3 or more dep.
NOTE: IF YOU DISAGREE WITH YOUR ASSIGNED BENEFIT AMOUNT, YOU MUST APPEAL WITHIN 21 DAYS!
8. If you do not have the necessary paperwork from the state, YOU ONLY HAVE, SIXTY DAYS TO FILE FOR SUB. Contact the Benefits Dept to VALIDATE a SUB application so that you can still be timely.
Sub Benefits: SUB fund will pay $300.00
9. Attach your state information to your SUB application and drop off at the Benefits Lobby or mail to: International Truck and Engine Corp. ATT: Benefits Dept., 6125 Urbana Rd. Springfield, Ohio 45502. PLEASE WRITE CLEARLY!!!
